Our personal consultations and fittings are done by appointment only. Please call our office to schedule an appointment with one of our Breast Care Specialists.
If you want us to submit your claim to your insurance, we will need a physician’s prescription for your products before they can be dispensed. You may bring the prescription in with you at your appointment along with your insurance cards and photo I.D. or you may have your physician’s office fax your prescription directly to us at 815-578-0343.
Our office hours are Monday 12:00 to 4:00 and Tuesday — Friday 9:00 to 4:00 with evening hour appointments available Monday and Tuesday upon request.

HOME FITTING SERVICES AVAILABLE FOR THE HOME-BOUND AND SENIOR LIVING RESIDENCES
We offer a Home Fitting Service for women who are homebound or do not have transportation to come to us. Please call our office to schedule a Certified Fitter to come to your home for your personal fitting.
HOW TO RE-ORDER
If you are interested in reordering the same products you received from a previous fitting appointment with us and are confident you have not changed size, for your convenience you may call in your order. If you want us to file the claim with your insurance, we will need a new physician’s prescription. This may be faxed to our office 815-578-0343. Once we receive the prescription we will ship your order along with the required forms that need to be signed and mailed back to us so that we can submit it to your insurance.
